ACONE Lending Library Request Form

Adoption Community of New England, Inc. loans books, audio tapes, videotapes, and DVDs from the ACONE Lending Library via the mail to its members at no charge. Search the ACONE lending library.

How to Borrow

You may borrow up to two titles at a time with a refundable deposit of $50.00. You may use Visa, MasterCard, American Express or Discover for your deposit, or you may use a personal check made out to ACONE.
The loan period is one month, unless otherwise stipulated.
Your request will be processed as soon as your choices become available. The borrower is responsible for return postage.
If this loan request accompanies a return, you will need to send a new deposit check. Your old deposit check will be returned with the new loan. If you used a credit card for your deposit, the same credit card number will automatically be used for your new request.
Complete and mail or fax your request to the location provided on the bottom of the form.
